You will need the following documents when registering:

• if you already have a child registered with the school board, you will need their NISO number, located on one of their report card to register the new child
• a proof of your residence in Ontario (e.g. recent statement of your gas or electricity account)
• a proof of the child's civil status (e.g. birth certificate, Canadian passport, Canadian citizenship card, request for refuge or other official immigration document)
• the legal documents relating to access to the child's data, if applicable (e.g. court order or custody judgment)
• a proof of the child's catholicity: baptismal certificate
• a proof of immunization: immunization card (record of vaccines and inoculations); please contact the local health unit if necessary.
